2020 application package download: Reinstatement of a Certificate of Practice (effective December 2, 2019)

This section applies to you if you previously held a certificate of practice with the College and are now applying to reinstate your certificate of practice.

What happens after I submit my application?

  • Applications are placed in a queue in the order in which they are received.
  • You will receive an email letting you know that we received your application. This will take at least 10 business days. 
  • Once you have met all reinstatement requirements, your application is forwarded for review.
  • Once the review is complete, we'll send a confirmation email.


All fees are non-refundable and non-transferable.
